Participation agreements

Voluntary participation and assumption of risk

I understand that football training is physically demanding. Risks include contact with people, equipment, and playing surfaces; falls; collisions; overexertion; heat or cold exposure; strains, sprains, fractures, concussion, other serious injury, permanent disability, and death. Other risks may be unknown or not reasonably foreseeable. I voluntarily choose participation for myself or authorize the minor participant to participate and knowingly assume the inherent and other disclosed risks.

Release of ordinary-negligence claims

To the fullest extent permitted by Colorado law, I, for myself and—if I am the participant's parent or legal guardian—on behalf of the minor participant, release Mostly Right LLC d/b/a Coach Polumbus, Tyler Polumbus, and their owners, employees, and authorized agents from prospective claims arising from ordinary negligence connected with the participant's training or presence at a session. This agreement does not release any claim that cannot lawfully be waived, including a claim based on a willful and wanton, reckless, or grossly negligent act or omission.

Safety and concussion response

I will ensure the participant follows instructions, uses required equipment, reports injury or unsafe conditions, and stops when directed. If a concussion or other serious injury is suspected, the participant may be removed from activity. The signing adult will be notified and should seek evaluation from a licensed health-care provider. Return to activity requires any clearance and graduated process required by law, the facility, and Coach Polumbus safety procedures.

Emergency authorization

If I cannot be reached promptly during an apparent emergency, I authorize Coach Polumbus to call 911, provide relevant contact information to emergency responders, and facilitate evaluation and treatment by licensed emergency personnel. I understand that Coach Polumbus does not provide medical care, does not guarantee a medical outcome, and does not receive authority for routine transportation through this paragraph. I accept responsibility for emergency-care costs to the extent permitted by law.

Parent-first communication and conduct

For a participant under 18, scheduling, agreement, payment, and safety communications go to the parent or legal guardian. Participants, adults, and spectators must follow facility and coaching rules and must not engage in harassment, hazing, bullying, threats, violence, sexual misconduct, retaliation, substance use, weapons possession, or other unsafe conduct. This agreement does not grant photography, video, publicity, testimonial, or marketing permission.
