Privacy Notice
This owner-approved notice explains how Mostly Right LLC d/b/a Coach Polumbus ("Coach Polumbus") handles information used for coaching inquiries, bookings, payments, required agreements, and service communications. Cal.com handles initial-session booking and Stripe handles its $175 USD payment.
1. Current website behavior
The fillable agreement form runs only in the browser tab. It has no submission endpoint, database, analytics, or local-storage feature. Information typed into it is not sent to Coach Polumbus. The print control opens the browser's print dialog so the signing adult may save or print a personal copy.
The initial-session booking buttons open Cal.com. Information entered there is submitted directly to Cal.com, and payment is processed through Stripe; this website does not receive payment-card details.
2. Information collection
The business and its approved providers may collect only information needed to provide services:
- Signing adult's name, email, mobile number, role, and signature record.
- Participant's name, age category, selected service, position, experience, and training goal.
- Emergency contact name, relationship, and mobile number.
- Booking, payment, attendance, cancellation, and agreement records.
- Messages and information a parent or adult participant voluntarily provides.
The website agreement form is designed not to collect a full birth date, athlete email or phone number, detailed medical history, government ID, insurance ID, or payment-card details.
3. Uses
Information may be used to respond to inquiries, determine training fit, schedule and deliver services, process payments through an approved provider, obtain and retain required agreements, communicate with the signing adult, address safety or emergencies, maintain business records, prevent misuse, and comply with law. Information should not be used for an unrelated purpose without a new notice or consent when required.
4. Children and parent-first collection
This service is directed to parents, guardians, coaches, and adult participants—not to children completing forms on their own. A parent or legal guardian must provide information and sign for a participant under 18. The form does not request the minor's own email or mobile number. A child under 13 must not submit any information directly through the website.
5. Sharing
Initial-session booking information is shared with Cal.com and payment information is handled by Stripe. Other information may be shared only as needed with approved providers that support electronic signatures, hosting, communications, professional advice, or legal compliance; with emergency responders when reasonably necessary; or when the signing adult directs or authorizes the disclosure. Coach Polumbus will not sell personal information or use it for targeted advertising.
6. Security and retention
A live workflow must use reasonable safeguards appropriate to the information collected, access limited to people who need it, and a written retention and secure-disposal schedule. Final retention, access, deletion, export, and incident-response details for Cal.com, Stripe, and WaiverForever remain pending. Website agreement submission remains disabled.
7. Choices and requests
A signing adult may request access, correction, or deletion where applicable, and may withdraw optional communications. Privacy questions and requests may be sent tocoachpolumbus@gmail.com or raised by phone at720-239-2416.
8. Updates
This notice should be updated when collection methods, providers, retention practices, communications, or hosting change materially. It does not claim attorney approval or legal compliance.
